HD Smile Saver Plan Disclosure
Membership fees must be paid in full before any benefits can be accessed. Payments must be made at the time of service; otherwise, standard fees will apply.
The HD Smile Saver Plan cannot be combined with dental insurance plans or other promotions. It is specifically designed for patients without dental insurance. Refunds are only available after the annual fee has been paid. The plan is valid for one year from the date of membership payment, with renewal rates subject to change.
Patients and responsible parties have the right to refuse, cancel, or request a refund for any payment made for services, examinations, or treatments provided within 72 hours in response to an advertisement for free, discounted, or reduced-fee services.
Additional charges may apply for related services required in individual cases.
Please note: This discount program is not a dental insurance policy and does not make direct payments to dental service providers. Members are responsible for all payments for dental services, and will receive discounts as part of the HD Smile Saver Plan. The plan does not cover treatment plans or fees incurred before the plan was purchased.
